Clinical Rationale

The options were fusion, a lifelong brace, or salvage. A custom implant was printed with a tunnel designed into it for a hamstring graft, anchored in the talus and sustentaculum tali — reconstructing both bone and ligament in one design.

Surgical Approach

The ankle was salvaged using a custom 3D-printed medial malleolus with a tunnel designed for passage of a hamstring graft. Two limbs of the graft were anchored — one in the talus, one in the sustentaculum tali — to reconstruct stability.

A near-lost medial malleolus from a compound road accident, salvaged with a custom 3D-printed implant and hamstring graft reconstruction instead of fusion or lifelong bracing.

A 21-year-old man sustained a severe open ankle injury in a road-traffic accident, requiring vascular repair and leaving most of the medial malleolus missing. The resulting ankle was painful and unstable because both bone and the medial ligament attachment had been lost. Fusion and permanent bracing were considered, but a custom reconstruction offered a chance to preserve the joint.
