Clinical Rationale
Once the talar blood supply is lost, joint-preserving options are exhausted. A custom implant replaced the necrotic bone and restored almost normal, pain-free movement — with the Centre's cases now surviving up to six years.

Surgical Approach
A custom total talus replacement was implanted, replacing the necrotic talus and restoring the ankle's articulation. The patient regained almost normal, pain-free range of motion post-operatively.

A 32-year-old woman developed avascular necrosis of the talus, a condition in which loss of blood supply causes the bone to weaken and collapse. Pain and progressive damage threatened both the ankle and hindfoot joints. A total talus replacement was considered as a motion-preserving alternative to a large fusion.
