Clinical Rationale
Pilon, talus and calcaneus fractures on both sides, alongside head and abdominal injuries, raised a real question of whether the patient would walk again. Both hindfeet were fused with a custom straight nail that compresses the ankle and subtalar joints simultaneously.

Surgical Approach
Both sides were reconstructed with tibiotalocalcaneal (TTC) nailing; the left also received primary bone grafting. The centre's own straight nail design allowed nail-assisted compression across both the ankle and subtalar joints, achieving sound union despite severe comminution.

A construction-site polytrauma with bilateral pilon, talus and calcaneus fractures, salvaged with bilateral TTC nailing using a custom in-house compression nail.
A 37-year-old man sustained devastating injuries to both ankles after falling at a construction site. The trauma involved the pilon, talus and calcaneus on both sides, together with head and blunt abdominal injuries. He reached the foot and ankle team three weeks later with both legs supported in below-knee plaster slabs and with very limited reconstructive options.
