Clinical Rationale

The talus was extruded entirely in a road accident and later brought in by relatives — too contaminated to replant. A CT scan of that extruded bone was used to reverse-engineer a patient-specific titanium replacement, implanted once infection risk had passed.

Surgical Approach

After debridement, an antibiotic cement spacer and external fixation, the decision was made not to replant the contaminated talus. A CT scan of the extruded bone was used to 3D-print a patient-specific titanium prosthesis — India's first 3D-printed total talus replacement — implanted via an anterior approach at three months.

A 39-year-old man sustained a severe open ankle injury in a road-traffic accident, with the talus completely extruded from the body and multiple associated injuries.
