Clinical Rationale
Rather than operating on an already-inflamed tendon, the calcaneus was rotated to move the Achilles away from the Haglund prominence and alter its line of pull. Tension dropped, the spur was shaved percutaneously, and the patient was fully mobile by nine weeks.

Surgical Approach
A minimally invasive dorsal closed-wedge osteotomy of the calcaneus (Zadek's) was performed. The osteotomy rotates the Achilles away from the Haglund prominence and alters its vector of pull to reduce tension, without touching the tendon itself; the bony prominence was shaved percutaneously with a burr.

A minimally invasive Zadek's osteotomy for two years of insertional Achilles pain — reducing tendon tension without ever touching the tendon, fully mobile at nine weeks.
A 41-year-old woman had experienced posterior heel pain for two years because of insertional Achilles tendinopathy. The painful tendon insertion and associated bony prominence made footwear and daily walking uncomfortable. A minimally invasive Zadek osteotomy was selected to change the mechanics of the heel without directly cutting into the diseased tendon.
