Clinical Rationale

In a neuropathic foot, large incisions invite the very complications you are trying to avoid. A percutaneous osteotomy at the first metatarsal base offloaded the pressure point driving the ulcer, allowing a wound of two years to finally close.

Surgical Approach

A burr-assisted dorsal closed-wedge osteotomy at the base of the first metatarsal was performed percutaneously to offload the pressure point, with the osteotomy closed to correct the deformity — all through minimal exposure.

A patient with diabetic neuropathy had a plantar ulcer beneath the first metatarsal head that had failed to heal for two years. Loss of protective sensation allowed repetitive pressure to continue through the same area with every step. Instead of repeatedly dressing the wound alone, treatment focused on removing the mechanical pressure responsible for keeping it open.
